Broad Statement Responsibilities :
Provide direct care to adults who qualify for services under the MaineCare Eligibility Manual. Responsibilities will include but are not limited to, helping support the client in achieving their personal goals as defined in their individualized support plan. Ensure that the client can access the community in a way that promotes independence and maintains safety. Must enjoy working with people and have a desire to help individuals maintain the highest level of independent living in a safe environment.
Qualifications:
Education : High School diploma or GED
Experience: One year of experience working with adults in a supervised setting or relevant life experience.
Certification: Staff must be committed to earning their Mental Health Rehabilitation Technician-1 within one year of their date of hire. Staff must complete CPR and First Aid Certification.
Position Duties and Responsibilities:
Provide direct care (behavioral health services) as outlined in the Individual Service Plan focused primarily on:
Helping the client achieve the objectives on their plan;
Provide information and support to the client;
Ensure the client has access to specific services, supports, and resources identified in the plan; and
Observe and report significant changes in the condition and needs of the client to the immediate supervisor.
Report to immediate supervisor and/or program manager all suspected cases of abuse or neglect and document appropriately.
Maintain and submit documents as required by agency policy and regulatory requirements. This includes all progress notes on a weekly basis by the designated time as set forth at orientation.
Support client during periods of crisis by consulting with supervisor and program manager when needed, referring to crisis intervention services and or 911 when necessary, and completing agency incident report as indicated.
Maintain confidentiality in accordance with agency policy and regulatory requirements.
Perform additional duties as assigned.
General Requirements :
Must be 18 years of age.
Reliable transportation, valid driver’s license and liability insurance. All employees must pass the motor vehicle record check conducted directly with the Department of Motor Vehicles.
Good verbal and written skills.
Ability to present a professional image appropriate to job responsibilities.
Well-developed organizational skills and ability to carry out duties with limited supervision.
Ability to fulfill assignments with reliability and punctuality.
High moral and ethical values as set forth in the SequelCare of Maine Employee Handbook.
Willingness to be open to learning and growing.
Appropriate interpersonal/personal boundaries and the ability to work cooperatively as a team member.
Maturity of judgment and behavior.
Ability to remain flexible to reasonable changes in the work schedule.
Must complete CPR and First Aid certifications.

Specific Physical Requirements for Position: Walking, squatting, bending, kneeling, reaching, twisting, crawling, pushing, pulling, lifting and carrying up to 25 pounds, the work environment that is typical for working with adults in their home and the community.
