Social Worker Administrative Support - Lakewood - Waterville, ME
The position is responsible for providing support to the social services department in accordance with the Lakewood Mission, Vision, and Values in order to enable residents to attain or maintain the highest practicable physical, mental, or psychosocial well being.
Essential Functions and Work Role Responsibilities
1. Understands the rights of the resident and the resident’s family.
2 Makes referrals to obtain services from outside agencies (eg: hospice,) as needed.
3. Makes arrangements to obtain adaptive equipment, clothing, and personal items as needed.
7. Arranging for post discharge supportive services (ie: home care, therapy, etc); making arrangements for supplies and equipment; and other related duties.
8. Initiates Medicaid “Days Awaiting Placement” applications and provides documentation and follow-up as needed.
9. Arranges for the provision of counseling services as needed.
10. Assists residents with financial and legal matters such as applying for Medicaid, contacting Social Security, and making referrals to attorneys.
11. Assists with tracking medical eligibility assessment schedule and initiates referrals to the appropriate resident medical assessment contractor (Goold Health Systems).
12. Obtain information for completion of MDS assessments
13. Performs other related duties as assigned by the Director of Social Services.

1. Maintain a safe environment which includes knowing and understanding facility and departmental policies and procedures.
2. Report and directly address identified environmental hazards when appropriate.
3. Report and directly address violations of resident safety policy and/or protocol when involved or observed.
Physical Environment
Works in a multitude of public and private locations. Primarily works in a private office or in a public health care location. May be exposed to occupational hazards/diseases in private homes, hospitals, and other patient care settings.
Physical Criteria
Physical effort minimally required. Some duties may require standing, wide freedom of movement, stooping, bending, lifting, pushing or pulling (25lbs). Frequent periods of concentration requiring close attention to detail.
Occupational Risks
Failure to follow established procedures could result in undue exposure to safety hazards and/or disease/infection in resident contact.
Qualifications
Prior health care experience preferred. Ability to work well with others and to function independently, without supervision. Ability to prioritize and handle multiple tasks simultaneously. High school diploma required, Associates Degree in Human Services, or higher, preferred.
