Day Program Case Manager - LifeShare Management Group LLC - Waterville, ME

LifeShare Management Group LLC, a growing human services agency serving individuals of all ages and abilities, is currently searching for a Day Program Case Manager!

As the Day Program Case Manager, you will be responsible for the day to day operations of LifeShare’s community-based day program including, but not limited to:

* The recruitment, development, retention and scheduling of Direct Support Professionals.

* Attending any and all team meetings connected to the person receiving support.

* Maintaining required documentation including, but not limited to attendance, incident reporting and monthly progress notes.

* Regular site visits to ensure that each program assigned to caseload is in compliance with state regulations. This includes preparing for and attending each state certification review.

* Ongoing direct support including personal care assistance and crisis intervention as well as 24-7, on-call coverage.

This full time position with weekday office hours is based in Waterville, ME, however, requires travel throughout the state. Nights and weekends may be required for scheduled events/visits. Travel is reimbursed at the current IRS rate.

The successful candidate must possess a bachelor’s degree in human services, social work or related field along with a minimum of two years relevant experience.
