Admissions Coordinator - Lakewood - Waterville, ME

Summary of Major Functions

The position is primarily responsible for the planning, coordination, and direction of all facility admissions. The Admissions Coordinator works in collaboration with Nursing, Social Services, Rehab Services, and the Business Manager to assure that all patients admitted to the facility meet medical and financial eligibility criteria. The Admissions Coordinator works extensively with physicians, discharge planners, nurses and other hospital staff to assure accurate information flow and to assure smooth transitions between hospitals and the facility.


Essential Functions and Work Role Responsibilities


1. Actively seeks, receives, and tracks resident/patient referrals through development of community network links.


2. Maintains database of referral/admission information and analyzed monthly to identify changes in referral patterns.


3. Collects data from referral sources to determine eligibility status (financial and medical) for admission to the Skilled Care Unit or Nursing Facility.


4. If Medicare, contact business office to determine financial eligibility. If Medicaid, contacts the appropriate person(s) to determine financial eligibility. Along with determining financial eligibility, medical eligibility is also determined and documented.


5. Verifies and documents eligibility and benefits if patient has a private insurance company


for financial coverage. (If private insurance is secondary to Medicare, verifies the same.)


6. Performs pre-admission, on-site assessments of prospective residents/patients.


7. Verifies that all the necessary admission documents are complete, accurate, and documented prior to admission in ECS, the Electronic Charting System .


8. Through community contacts, works to maintain a high census and budgeted occupancy.


9. Represents the facility in the community and, where appropriate, develops a community education program, and is available for tours of the facility.


10. Responsible for facility marketing and public relations.


11. Participates as appropriate in the quality improvement process.


12. Stays up to date on all rules and regulations governing admissions into NF or SNF care.


Physical Environment


Works in a multitude of public and private locations. Primarily works in a private office or in a public health care location. May be exposed to occupational hazards/diseases in private homes, hospitals, and other patient care settings.


Physical Criteria


Physical effort minimally required. Some duties may require standing, wide freedom of movement, stooping, bending, lifting, pushing or pulling (50lbs). Frequent periods of concentration requiring close attention to detail.


Occupational Risks


Failure to follow established procedures could result in undue exposure to safety hazards and/or disease/infection in resident contact.


Senior Management supports supervisor safety and loss control training. Will be held accountable for injury prevention, accident/injury investigation to include root cause analysis and assist in the return of all workers to gainful employment.


Safety


1. Maintain a safe environment which includes knowing and understanding facility and departmental policies and procedures.


2. Report and directly address identified environmental hazards when appropriate.


3. Report and directly address violations of resident safety policy and/or protocol when involved or observed.


Qualifications


Valid driver’s license. Prior health care experience in either a clinical or administrative capacity. Current, valid State of Maine RN license is preferred. Ability to work well with others and to function independently, without supervision. Ability to prioritize and handle multiple tasks simultaneously. Background in sales or marketing helpful.
