Case Manager - Maine Behavioral Health Organization - Skowhegan, ME

Title : Community Integration Services Case Manager, Mental Health Services .

Function : The Community Integration Services Case Manager is a trained, supervised professional who works as a member of a team of professionals and provides mental health services. Services involve direct contact with recipients of services, assistance in symptom management and advocacy and assistance with access to resources. The Community Integration Services Case Manager works independently with a high level of autonomy while coordinating services with other members of the Mental Health Team. This position requires work in clients’ homes, school, and with the community.


General Expectations : § Commitment to the mission of the Agency. § To understand and work within Maine Behavioral Health Organization (MBHO) Personnel, Accounting, Administrative, and Service Area Policies and Procedures. § Work in harmonious relationship with all agency staff and community.

§ To accept shared responsibilities and authority with other team members to successfully accomplish the goal of each team of which s/he is a member. § Perform duties in a conscientious and cooperative manner. § Perform required amount of work in a timely fashion with minimum errors. § Assure quality in work performed in order to facilitate the delivery of quality services. § Be punctual for scheduled work and use time off in accordance with policy and protocols. § To follow NASW Code of Ethics, and, inasmuch as they are congruent with these, to follow applicable professional codes of ethics. In addition, to comply fully with all relevant licensing, certification, accreditation and legal standards as required by the agency, contracts, Medicare, Medicaid (MaineCare), HIPAA, and other applicable federal, state, and local laws and regulations.


Primary Responsibilities : The primary responsibilities of this position include, but are not limited to, the following: § Work collaboratively as a member of a team with various groups of staff depending on the issue addressed.

§ Determine consumer eligibility for targeted case management by performing a comprehensive psychosocial assessment and a yearly assessment update.

§ Plan with the consumer and Clinical Supervisor to deliver appropriate community support services as identified in the assessment.

§ Provide direct services to clients that include, but are not limited to: emotional support, assistance with problem solving, training in ADDs , encouraging medication compliance, monitoring behavior, advocacy, and assistance in obtaining comprehensive and coordinated services in the community.

§ Provide 90- day review of the service/support plan in conjunction with the client and Clinical Supervisor.

§ Document in the case record all client related information and events, including assessment, service/support plan, 90- day reviews, narrative notes, and progress notes.

§ Demonstrate stability and the ability to produce satisfactory work under pressure, heavy volume, and/or tight deadlines, maintaining self-control and coping with the unexpected.

§ Participate as a member of the Community Support Team including Mental Health Program Manager, Clinical Supervisor, and other service provider agencies. This team will include the client and family members (as allowed or desired by client).

§ Participate in continuing education as required by the agency, Department of Health and Human Services standards.

§ Accept and perform other duties as assigned.

Staff Reporting Relationship : The Targeted Case Manager reports directly to the General Director; Clinical issues to the Clinical Director
