Relief Medical Interpreter - Chinese - Stanford Health Care - Palo Alto, CA

Under the direct supervision of the Manager of Interpreter Services, conducts medical and general interpreting/translating for the patient care staff and for patients and their families. Acts as a cultural consultant to staff and as advocate for non-English speaking patients/families. Assesses and monitor their patient's understandings of the information conveyed and keeps physicians/nurses informed as to the level of that understanding. Suggests referrals to medical staff, clinical social workers, patient relation staff, chaplains, and ancillary staff. Records services provided in patient chart.

Registered Clinical Dietitian - Diabetes Program (Per Diem - Varied Shift) - Sutter Health - San Francisco, CA

This position provides outpatient Diabetes self-management education for individuals and group classes according to the ADA National Standards. This position is responsible for development and instruction of individualized meal plans that promote treatment goals that include glucose and lipid normalization, weight loss and hypertension control. This position also performs nutritional assessment of Diabetes Center outpatients in a manner that considers medical history, mental and physical state, and religious and cultural practices. Provides diabetes nutrition counseling that is consistent with patient and treatment goals, and is appropriate to an individuals learning style.

Communicates significant findings and care plan to other diabetes team members, hospital staff, and primary care physicians. Communicates in a proactive manner with physicians regarding patient needs in order to optimize diabetes management and promote established clinical standards of care. Develops and collaborates with other diabetes management team members in the development of patient education materials, care maps, care algorithms, and lesson plans for individuals and groups. Uses critical thinking to develop and continually improve processes, program offerings, and clinical practice. Provides group instruction in nutritional planning and diabetes self-management skills. Uses individual strengths to enhance overall performance of the diabetes team. Provides clinical expertise to hospital dietary and nursing staff and physicians in current nutritional guidelines and recommendations regarding diabetes management on an as-needed-basis or through formal professional education programs.

Qualifications

Minimum Bachelor of Science in Nutrition required. Dietetic Internship required. Minimum 3 years in Diabetes Education. Current Registered Dietitian License required. Current Diabetes Education Certification required or recent experience in Diabetes Education, ongoing continuing education in diabetes and working toward CDE Certification. Flexible/works well with people. Spanish or Chinese speaking desirable. Insulin Pump Training experience desirable.

Ability to organize to maximum number of patients. Work effectively and positively with other employees and patients. Maintains commitment to professional growth by participating in educational opportunities and/or professional organizations. Has excellent customer service skills. Demonstrates commitment to the philosophy and values of Alta Bates Medical Center.

Covered California Plan Based Enrollment Counselors - Chinese Community Health Plan - San Francisco, CA

The Plan Based Enrollment Counselors help enroll new or exisitng members for renewal into the Covered California Program. The PBE's educates customers on how the health care system is organized, what services are available to them and provides enrollment referrals or assistance to public or private health/social services. The PBE's also serves as the Certified Application Assistor. The PBE's transitions to ongoing administration of the terms in the agreements with Covered California and facilitates annual renewals.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Ability to understand and explain to customers how health care is delivered in the community.

California certification
3. Provides strong customer service and support to potential clients,

Registered Nurse: Home Care

Job Summary:
A nurse currently licensed and registered in the state of practice, which provides professional services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates clients and their families. Professional skilled nursing is performed under the supervision of the Administrator, Director of Clinical Services (DCS) or Branch Manager.

Organizational Relationship:
Reports to the D.C.S., Branch Manager, or Administrator

Qualifications: (Preferred unless required by law)

Graduate of an approved professional nursing programCurrently, licensed “in good standing” in the state where skilled home visits will be provided.Minimum of one (1) year of experience as a professional nurse (may be waived at the discretion of the Administrator).

Responsibilities:

Receives the intake referral information and prepares paperwork/tools necessary for the visit.Obtains all pertinent medical history from client, family or significant others.Performs home safety check and environmental assessment of the client’s home environment.Performs the socio-psychological evaluation of the support systems available to the client and documents necessary emergency contacts etc.Performs assessment visit and documents accordingly (i.e. OASIS/ Skilled Nursing Note etc.).Performs physical examination and review of all body systems and documents such accordingly.Develops an appropriate and effective Plan of Care (POC) to be submitted to the physician for approval and implementation.Determines medical necessity for other services that could enhance the positive outcome desirable for the case.Evaluated the client’s ADL and IADL abilities and therefore need of support services such as home health aide.Develops and implements the HHA plan of care when HHA services are ordered. Revises and signs this care plan the beginning of each certification period.Supervises the HHA every 2 weeks (in accordance with state/federal requirements) and documents the supervision without having to be directed to do so.Orders “other” professional services that are effective and is willing adjust frequency of these services as outcome progresses and the client’s condition warrants.Reviews billing processes with client and/or family advising client and/or family when co-pay or Medicare is not likely to pay for services (ABN).Effectively communicates with client and family the POC and progression of such. Keeps the client informed ongoing.Effectively communicates with other disciplines in the case (case conferencing) to effectively and appropriately problem solve as situations arise.Communicates effectively with the DCS or Nursing Supervisor scheduled visits planned and changes to the schedule on a weekly basis. Caseload is self scheduled but communication of the clinician’s schedule is essential.Communication with the client’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities to effect the best means to obtain the desired outcome for the specific medical problem that caused the case to open and/or recertify.Communicates in the case conferencing sessions to establish best practices for the individual client’s needs.Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.Submits ALL OASIS documentation within 48 hours of the OASIS visit, WITHOUT exception.Submits requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring.Coordinates Community Services that may be available to the client to assist in safe home care needs.Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office. Assures that visits are not done that cannot be billed.Participates in the Performance Improvement Committee process as requested to do so.Performs at least 5 clinical record reviews (CRR) per month in collaboration with the Nursing Supervisor. Submits the CRR timely each month.Participates in the monthly staff meeting as part of the requirements of the position. Adjusts visits on the staff meeting day to accommodate this important function.Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees.Participates in the planning, operation and evaluation of the nursing services of the organization.Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the clients serviced.Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the client caseload as quickly as possible relative to the event’s occurrence.Submits written time requests 2 weeks or more in advance of planned time off.

Utilization Management Coordinator - Chinese Community Health Plan - San Francisco, CA

Position Summary

Provides assistance to CCHP Medical Management Department in all stages of authorizations, tasks, and work flows.

Major Responsibilities and Duties

1. Verifies member eligibility, enrollment, Evidence of Coverage, plan codes, and Explanation of Benefits.
2. Prepares and completes authorization forms for review.
3. Assures timely, organized processing of inpatient and outpatient authorizations for input into the core computer system.
4. Utilizes fax and telephone contacts to obtain information (clinical and non-clinical) from physicians, families and facilities and reports information to UM nurses/medical director requested.
5. Processes denials for non-coverage ensuring that the proper letterhead, template and version is used and the proper denial language is used in compliance with current regulations, policy and procedures.
6. Serves as back up to all additional Utilization Management Assistants.
7. Scans authorizations, notices of non-coverage and other UM documents for filing.
8. Complies with CCHP Compliance Handbook including Code of Ethics and all statues, regulations and guidelines applicable to federal and state programs. Responsibilities include following guidelines, reporting suspected violations of any statute complaints with regulations, agreements or guidelines applicable to all health programs or TPA programs.
9. Accepts and performs other duties as assigned.
10. Performs audits on outpatient utilization as requested.
11. Files all related authorization documents in appropriate paper or electronic folders.
12. Multitask effectively (enter authorizations, complete denial letters, filing, phone answering, reports as requested/required, correspondence letters and additional duties as assigned).

Overall Statement:

Global Health Program (GHP) is one of SIDO’s programs of which many projects were designed under its program component within the organization, to be working in partnership with the Ministry of Public Health and Sanitation, Red Cross and the Ministry of Medical Services to strengthen Public health, Sexual and Reproductive Health (SRH), HIV/ TB, and Malaria Prevention, Care and Treatment services and other related health projects in each country consisting with the regional priorities and initiatives. This program is funded by Private Funds, Dalux Foundation and other donors.

GHP is currently accepting expressions of interest from candidates for a prospective funded project of the role of Chief of Party (CoP). The CoP will be responsible for strategic direction and management of a complex national training project to increase the number of new health workers trained, address workforce gaps in skills and knowledge, build the capacity of training institutions and strengthen regulatory bodies to increase demand for training. Successful candidates should demonstrate high-level expertise in human resource management and health systems strengthening with at least 5 years of experience managing health projects in a resource-constrained country.

Responsibilities: • Provide strategic direction and overall management of the project; Provide oversight of planning and management of the project annual work plan and budget and Provide oversight of administrative and financial management; • Provide oversight of human resource management; Provide oversight of project monitoring and evaluation; Coordinate all public relations and communications; • Communicate with donors and Ministries of Health/National AIDS and STI Control Program; Prepare required reports on project activities; Represent the project in meetings with donor, government officials, and partner organizations; • Plan, organize and direct all activities associated with the project; Manage relationships with the donor, government agencies, local authorities, and other agencies and contractors; Coordinate the production of timely progress reports on the project; • Ensure completion of all grant objectives and report regularly to the donor; Supervise project staff; and Handle fiscal management of the project;

Qualification and Experience: • Master’s degree (or PhD) in a health related, International Development or Management field. • At least 10 years of professional experience in international development, including at least five in developing countries, 5 years of experience in managing a complex public health project (RH and Family Planning and HIV project an advantage). • Experience in training, mentoring and supervision of health care workers. • Knowledge of in-country health system. • Familiarity with PEPFAR and other Private Funding initiatives. • Experience working with persons with disability and/or a solid working understanding of disability-related social and health issues a plus. This should include an understanding of the field of social work and case management, as well as experience in policy development. • Knowledge of community-based approaches for designing and delivering services. • Experience in managing private funded projects highly desired. • Demonstrated capabilities in institutional capacity building, high-level strategic visioning and leadership, and experience in working effectively with civil society, local, regional and central-level government authorities and with Foundations and Private Funds or other development partners. • Proven exceptional leadership in the design, management, implementation, monitoring, and evaluation of projects funded at a minimum of $1 million per year, with skills in strategic planning, management, supervision, and budgeting, and preferably experience in managing complex activities involving coordination with multiple project partner institutions. • Strong communication skills, including interpersonal, to fulfill the diverse technical and managerial requirements of the projects and to effectively coordinate with a wide range of country/ regional stakeholders, Excellent written and oral communication skills essential. • Proven ability to develop and communicate a common vision among diverse partners and the ability to lead multi-sectors (such as public and private sector) and multi-disciplinary teams. • Experience working in assigned country desirable, but not necessary. • Language: fluency in written and oral English a MUST, a basic local language an advantage.

Salary, Benefits and Conditions: This is an accompanied position and the salary is commensurate with experience. Annual gross range: US $ 45K – $ 50K (Plus benefits). SIDO - GHP offers a generous benefits package including 50% educational cost for children under 18 yrs, housing & reallocation, medical, dental, vision, short/long term disability insurance, life insurance, 403b (group retirement annuity) and partial subsidy of public transportation cost.
