Behavioral Health Coordinator LCSW job - MaineGeneral Health - Oakland, ME

Under the general direction of the Medical Director, Practice Administrator and Clinical Coordinator, the Behavioral Health Coordinator is responsible for providing and overseeing behavioral health services to patients at the primary care practice. As part of an integrated behavioral health and primary care practice, the Coordinator performs patient screenings and assessments; provides counseling, information, referrals and skills training; crisis management; care coordination and follow-up.

Qualifications/Competencies:
Current Maine State Board of Social Work Licensed Clinical Social Worker (LCSW) is required.

Master’s degree as an MSW in Counseling or a related field preferred .

3 years or more of dedicated work experience with behavioral health patient population is strongly preferred . Proficiency in a suite of Microsoft Office applications. Ability to competently organize, facilitate, educate, document, and work effectively with patients, families, physicians, managers, administrators, community contacts and other disciplines. Consistently use good judgment in dealing with all internal and external customers. Ability to use electronic medical records system. Ability to work independently, effective time management and problem solving skills, able to manage multiple priorities . Detail-oriented, with strong organizational skills and ability to multitask . Must have confident and effective oral and written communication skills. Ability to react calmly, objectively and effectively in emergency situations.
