Community Integration Worker job - Emdeon - Augusta, ME





The CIW will provide case management, support and community integration services that involve the identification, assessment, planning, linking, monitoring and evaluation of services and supports needed by members.

The CIW will collaborate with the nurses and LCSW and monitor the Individualized Support Plans done by CIW in the community. They will work with other state agencies for efficacy in conjunction with members and collaboratively update and revise plan as needed.

They will provide information, consultation and problem solving assistance and support to members and family or members of support system including providers. They will facilitate and coordinate team meetings as appropriate. Perform other duties and responsibilities as may be assigned from time to time. Travel reimbursement will be at the current State rate of $.44 cents per mile.

Six (6) year combination of education, training, and/or experience providing case management services. Applicants must possess a MHRT-C certification.
