Social Work Care Manager – Bi-lingual Russian
PRIMARY RESPONSIBILITY:
Responsible for the initial psychosocial assessments and reassessments for clients with multiple
medical and psychosocial problems. Also responsible for enrolling referred clients. Provides case
management services for a specified client case load ensuring provision of quality of care.
ESSENTIAL JOB FUNCTIONS:
Conducts comprehensive psychosocial assessments, identifying important health (mental and physical), psychosocial, environmental and spiritual problems of the client and existent support systems.
Determines client’s functional level based on findings from the social work assessment.
Writes social work assessments, consistent with the guidelines in the MSSP manual, which are client-centered, comprehensive, timely and legible.
Identifies appropriate community services with a good knowledge of Medicare, Medi-Cal, and other entitlement programs.
Initiates original service planning, documenting purchased services according to MSSP guidelines. Verifies/authorizes and signs SPUS according to site policy on a monthly basis.
Makes home visits as needed for team’s clients. For his/her caseload, visits at least every three months, contacts clients monthly and reviews the service plan.
Establishes and maintains a therapeutic relationship with clients; treats all clients and their support systems with respect and dignity; provides supportive, empathic, and consistent counseling and teaching; sets limits and maintains professional distance; confronts the important problems and follows through with therapeutic interventions in a timely manner. Provides crisis intervention and advocacy when necessary.
Participates in annual peer review and staff evaluations according to site policy.
Other duties as assigned.
Education: Master’s degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology from an accredited school preferred; Bachelor’s degree required. Training in gerontology/geriatrics preferred.
Background and Experience: Required - A minimum of one year working with the elderly.
Required - Bilingual in Chinese.
Demonstrates understanding of normal aging and health care of older adults, case management, rehabilitation and health issues relevant to the frail elderly, e.g. family systems, alcoholism, dementia, personality disorder, etc.
Demonstrates understanding of the health and psychosocial needs of the frail elderly.
Demonstrates knowledge of community resources available for this population.
